Reduce the volume of services requiring PA by January 1, 2026
Honor existing authorizations during insurance transitions
Expand real-time approvals for most requests by 2027

Improve transparency around decisions and appeals

Fast forward to April 2026: AHIP reported that leading health plans eliminated 11% of prior authorizations across medical services — representing 6.5 million fewer PA requests. One large national plan committed to eliminating authorization requirements for 30% of healthcare services entirely.

The regulatory backstop is also in place. As of January 1, 2026, CMS-regulated payers (Medicare Advantage, Medicaid, CHIP, Marketplace) are required to respond to PA requests within 72 hours for urgent requests and 7 calendar days for standard requests. Miss those windows and you're non-compliant.

Expect your implementation conversations with these vendors to get more specific about ePriorAuth readiness over the next 12–18 months.

Physician practices

The 2027 mandate applies to your payers, not to you — but you're the downstream beneficiary. If your major payers (UHC, Aetna, Cigna, Elevance) are building FHIR-based ePriorAuth interfaces, your practice management system should eventually be able to submit and receive PA decisions without a single fax or portal login. Start asking your PM vendor where they stand on FHIR-based PA integration.

Denial management teams

The 30% reduction in PA volume at one large national plan is significant. Fewer PAs means fewer PA-related denials — but it also reshuffles the denial mix. You may see a higher concentration of denials in the remaining high-scrutiny service lines. Update your tracking to identify which service categories still carry PA requirements after January 1, 2026 cutoffs take effect.
